A user approves the correct number matching prompt, but the sign-in fails immediately afterward.

Recommended Resolution Path

  1. Inspect the Entra sign-in log to see which policy stage actually blocked the request.
  2. Check device compliance, risk policy, and location conditions rather than assuming MFA itself failed.
  3. Confirm the user approved the prompt from the correct device and account.
  4. If repeated denials occur with a valid prompt, collect correlation IDs before escalating.
